How to practice GLP-1 Receptor Agonists

Prescribed for type 2 diabetes, obesity, or metabolic syndrome. Typical dosing: once-weekly injection (semaglutide or tirzepatide) or daily injection (liraglutide). Combine with diet and activity modification. Common side effects: nausea, GI upset, constipation. Avoid in history of medullary thyroid cancer or pancreatitis. Not yet approved for cognitive prevention.

The evidence behind GLP-1 Receptor Agonists

Large RCTs confirm strong cardiometabolic benefits - weight loss, improved glucose control, and reduced cardiovascular events. While early studies suggested slower cognitive decline via enhanced insulin signaling and mitochondrial function, the large Phase 3 EVOKE trials did not meet their primary endpoint of slowing cognitive and functional decline in patients with established Alzheimer’s disease. Mechanism: The intervention restores neuronal glucose utilization and dampens inflammatory pathways, yet this biological effect did not translate to clinical benefit in later disease stages over the trial period. Evidence remains strong for metabolic and vascular protection, which is crucial for prevention, but is now lower for direct cognitive impact once the disease is established.
