Mechanism involves enhanced mitochondrial respiration, reduced oxidative stress, and inhibition of tau aggregation. Early small trials showed mild cognitive benefits, but large phase 3 trials (MADE, LUCIDITY) failed primary endpoints. Evidence mixed: biological plausibility remains high, but clinical benefit remains unproven. Risks include serotonin syndrome with SSRIs or MAOIs at high doses.
