Dr. Matthew Walker on DORAs and Alzheimer's Prevention

In this episode of The Matt Walker Podcast, Dr. Matthew Walker, Professor of Neuroscience at UC Berkeley and author of "Why We Sleep," explores groundbreaking research on a new class of sleeping medications and their potential role in Alzheimer's prevention.

Bottom Line: The brain's glymphatic system clears Alzheimer's proteins during deep sleep. Insufficient sleep creates a vicious cycle of protein buildup and worsening sleep. A new class of sleeping pills (DORAs) may enhance this cleaning process, though research is preliminary.

The glymphatic system: During deep sleep, the brain activates a cleaning system that specifically clears beta amyloid and tau protein, the toxic culprits in Alzheimer's disease. Without adequate deep sleep, these proteins accumulate.

The vicious cycle: Insufficient sleep allows protein buildup. These proteins then damage the very brain regions that generate deep sleep. Less deep sleep leads to more protein accumulation, creating a downward spiral that increases dementia risk.

Enter DORAs (Dual Orexin Receptor Antagonists):

Unlike traditional sleeping pills that cause sedation, DORAs work differently. They block orexin, the brain's master wakefulness switch, allowing for more naturalistic sleep architecture rather than simply knocking you out.

The preliminary research:

  • Animal and human studies show these drugs may reduce toxic proteins
  • Evidence suggests potential neuroprotective effects
  • Research is still early stage and requires larger, longer-term studies

FDA Removes Black Box Warnings on Hormone Replacement Therapy

Bottom Line: The FDA is removing black box warnings from HRT products after comprehensive scientific review. 

What happened: On November 10, 2025, HHS Secretary Robert F. Kennedy Jr. and FDA Commissioner Marty Makary announced the FDA is initiating removal of boxed warnings from HRT products following comprehensive review of scientific literature, an expert panel in July, and a public comment period.

Why the original warnings were misleading: The Women's Health Initiative study had important limitations:

  • Average participant age was 63 years—over a decade past typical menopause age
  • Used hormone formulations no longer in common use
  • Found only a statistically non-significant increase in breast cancer risk

What the science shows: Women who initiate HRT within 10 years of menopause onset (generally before age 60) are associated with:

  • Reduction in all-cause mortality
  • Up to 50% reduction in cardiovascular disease risk
  • Reduction in Alzheimer's disease risk (see Section 2)
  • 50-60% reduction in bone fractures

What's changing: The FDA is working with companies to remove references to risks of cardiovascular disease, breast cancer, and probable dementia from product labels. The boxed warning for endometrial cancer for systemic estrogen-alone products remains.

Additional actions: The FDA is also approving the first generic version of Premarin in over 30 years (improving affordability) and a new non-hormonal treatment for moderate to severe hot flashes.

What This Means: This FDA decision is important because millions more women will now have access to updated, evidence-based guidelines to help facilitate informed conversations about a treatment option that may significantly benefit brain health when started at the right time.

Hormone Replacement Therapy (HRT)

At BetterBrain, we consider HRT one of the highest-impact interventions for brain health in women navigating menopause.

The Science:Hormone replacement therapy (HRT) is commonly used to supplement declining hormone levels in post-menopausal women. Some hormones, like estrogen, appear to have a protective effect on the brain and may reduce inflammation and the accumulation of harmful proteins linked to neurodegenerative diseases, like Alzheimer's. By maintaining hormone levels throughout the critical menopause transition and beyond, HRT can reduce the risk of developing neurodegenerative disease.

Why We Prioritize HRT at BetterBrain

The evidence is compelling. While we need more randomized controlled trials, the observational evidence consistently points to significant brain health benefits when HRT is initiated during the menopause transition. The landmark Cache County Study found that women who used HRT for more than 10 years eliminated their sex-specific increased risk of Alzheimer's disease entirely—their risk became comparable to men's.

The impact can be substantial. The Cache County Study showed:

  • 41% reduced Alzheimer's risk among all HRT users (adjusted HR 0.59)
  • 67% reduced risk among former users who took HRT during menopause (adjusted HR 0.33)
  • Complete elimination of women's excess Alzheimer's risk (compared to men) with 10+ years of use

The window doesn't stay open forever. This is perhaps the most important point: the protective benefits appear strongest when HRT is initiated within 10 years of menopause onset, typically before age 60. Starting too late (in your 60s or 70s) may not provide the same cognitive benefits. The brain benefits from continuous hormonal support during the transition, not replacement decades later.

HRT isn't right for everyone. Decisions should be individualized based on:

  • Personal and family medical history
  • Risk factors for breast cancer, blood clots, or cardiovascular disease
  • Individual symptoms and quality of life
  • Personal preferences and values

What are exercise snacks: Think sprinting up stairs, doing jumping jacks, high knees, burpees or any movement that gets your heart rate up quickly in under a minute, performed periodically throughout the day.

About the research: Researchers analyzed 11 clinical trials involving 414 sedentary or physically inactive adults (69% women). The results?

  • Strong evidence for cardiorespiratory benefits in previously inactive adults
  • Time-efficient approach overcomes barriers like perceived lack of time
  • Less evidence for body composition or resistance training benefits: exercise snacks are specifically powerful for cardiovascular and metabolic health

Dr. Rhonda Patrick's take: Dr. Patrick is a big proponent of exercise snacks and goes into length about them in this video. She highlights research showing that just 3-4 minutes of these brief activities daily can reduce overall mortality risk by 25-30%. Increasing to 9 minutes daily can reduce cardiovascular mortality by 50% and cancer mortality by 40%.

BetterBrain expert insight: Our coaches recommend exercise snacks to clients who are inactive and want to get started. The evidence is very strong for cardiorespiratory fitness, especially if you're sedentary or relatively untrained. While they won't replace comprehensive strength training, they're an excellent entry point for brain health.

Here's how to integrate exercise snacks into your day for maximum brain health benefits.

The basic protocol:

  • Duration: 30 seconds to 2 minutes per snack
  • Intensity: Vigorous enough to spike your heart rate significantly
  • Frequency: 3 times per day, separated by 1-4 hours
  • Start with: 3 days per week, work up to 5-7 days

Simple options:

  • Stair sprinting (60 steps)
  • Jumping jacks (30-60 seconds)
  • High knees (1 minute)
  • Air squats (as many as possible in 60 seconds)
  • Burpees (10-20 reps)

Strategic timing:

  • Morning: Kickstart metabolism after waking
  • Mid-morning: Break up sedentary time around 10 AM
  • Afternoon: 2-3 PM to combat energy slump (better than caffeine!)

Breaking up sedentary time: Being sedentary is an independent risk factor for cognitive decline. Exercise snacks are particularly effective at countering the negative effects of prolonged sitting.

The stark findings (yes, they're as bad as you suspected):

  • All cohort studies found significant correlations between added sugar and cognitive impairment risk
  • Eight of nine cross-sectional studies showed the same pattern
  • Men consuming the most sugar were 23% more likely to develop mental disorders over five years
  • Higher sugar intake was linked to smaller brain volume and poorer memory performance

The key distinction: Natural fructose-containing foods (whole fruits) were actually associated with reduced cognitive impairment risk, while added sugars (high-fructose corn syrup, table sugar, etc.) were consistently harmful.

Plan for the 3-4 PM crash: This is when willpower dies and sugar cravings peak. Many of our members swear by keeping satisfying alternatives ready: mixed nuts, apple with almond butter, or a small piece of dark chocolate (70%+ cacao).

Timing matters more than you think: Having sugar after a workout? Your muscles will use it for recovery. Having it while stressed and sedentary? Straight to fat storage and blood sugar chaos. Context is everything.

  • Aim for 20–30 g protein within the first 1–2 hours after waking.
  • If you drink coffee, have protein with or before it. Caffeine can transiently reduce insulin sensitivity in some people, especially with a carb-only breakfast.

Why protein first matters:
Protein slows gastric emptying, blunts glucose excursions, and increases satiety signals such as GLP-1 and PYY. The net effect for many people is steadier energy and fewer mid-morning sugar grabs.

  • Hard-boiled eggs
